KS WebIZ Public Portal - Immunization Records Request

Need a copy of your vaccination record?

Kansas WebIZ now has a Public Portal!

https://myvaccinerecord.ks.gov/

You can now request your (or your dependents) immunization records to view or print off via the KSWebIZ Public Portal!

Additional Information:

 Kansas WebIZ is our state's Immunization Registry System which requires ALL vaccination clinics (in the state of Kansas) to enter immunization records into our State Immunization Registry System to reduce duplicate vaccinations.

In other words, any vaccine you receive anywhere in the state of Kansas (CVS/Walmart/Dillon's/Health Department, etc..) should be entered into this State Immunization Registry.

 Make sure you enter your LEGAL name. Ex: Katherine (not Kathy), or Robert (not Bob).

 Sometimes duplicate accounts are accidentally created due to name changes (married name, maiden name), spelling errors, etc...

We can merge accounts for you if you happen to have 2 accounts.

 If you are unable to find your Immunization Record, or believe there may be some immunization history missing, you can still call us at PCHD or you can contact the KDHE Immunization Registry at 877-296-0464 or email KDHE.ImmunizationRegistry@ks.gov
